Case managers and referral sources: call our intake line before sending a brokering packet. A named person takes ownership of the case on that call, tells you exactly where to send the packet, and confirms we can staff the client's ZIP code before anything is committed. Agate is an approved CCSP, SOURCE and ICWP provider, licensed by DCH in 47 Georgia counties.
