Celestina Aniekwe is a board-certified Family Nurse Practitioner with a Doctor of Nursing Practice from Chamberlain University and a Master of Science in Nursing from Bradley University. She brings broad clinical experience from major health systems including Grady Health System and Emory Healthcare, as well as primary care and in-home care settings across metro Atlanta.

Celestina specializes in geriatric care, chronic disease management, transitional care, and patient education in the home environment. She is committed to building trust with patients and families while improving health outcomes through compassionate, coordinated care.

She plays a key role in conducting internal audits, strengthening processes, and maintaining Agate’s strong commitment to delivering safe, reliable, and compliant care.

Aleena manages documentation, monitors regulatory requirements, and prepares the agency for state reviews and inspections, including those related to DCH oversight. Her attention to detail and dedication to excellence help protect clients, support caregivers, and keep Agate operating at the highest level of quality and integrity.
